You are required to use the Passport Wizard to complete your application.  Please bring your printed application to your interview. Handwritten applications will not be accepted. Making use of the Passport Wizard allows us to process passports more quickly, meaning they are returned to you earlier.

Be advised that forms must be completed and printed prior to your appointment.  If you do not bring the necessary completed form using the Passport Wizard, you will be asked to reschedule your appointment.  The only exception to this requirement will be if you are applying to replace a lost/stolen passport and are traveling within a week.
